About Us
We are a dedicated team advancing health justice by improving equitable access to palliative care, sexual and reproductive health (SRH), and prevention and management of non-communicable diseases (NCDs) for individuals, families, and communities. Working closely with community health promoters and local health systems, we identify, assess, and support people facing serious illness, chronic conditions, and unmet SRH needs. Our approach ensures timely, compassionate, and continuous care that responds to physical, psychosocial, and dignity-related needs—especially in underserved and hard-to-reach communities. Our work focuses on bridging systemic gaps in care delivery by bringing integrated services closer to where people live. Through community and home-based palliative care, SRH education and linkage, NCD screening and follow-up, psychosocial support, pain and symptom management, and strong referral pathways, we promote comfort, autonomy, and quality of life across the life course. We leverage innovative digital solutions, including the PCCare online platform, to strengthen care coordination, continuity, and accountability between communities and health facilities. Driven by a commitment to equity, dignity, and rights-based care, we empower patients, caregivers, and communities with knowledge, practical tools, and supportive networks. Together with our partners, we are building a sustainable, community-centered health justice model where no one is left behind in accessing palliative care, SRH, or NCD services and where every individual can live with dignity, support, and respect.
